His liver is massively enlarged – a condition called hepatomegaly – suggesting he had been a heavy drinker for many years. Chronic alcohol abuse causes the liver to accumulate fat, become inflamed and swollen, and often leads to cirrhosis, a severe scarring of liver tissue that impairs its function.
